A message from ISPAD's President

The COVID crisis has changed our lives dramatically. We have COVID-shutdowns, COVID-safe processes and COVID-anxiety. For some countries the peak of new cases is passing. We welcome the easing of lockdowns, when appropriate, and planning for the recovery phase.
 
In a number of less-resourced countries, LFAC and other NGOs are sending funds to help with purchase of food and other needs for T1D families in lockdown. Young volunteers with T1D, with police permits, are delivering insulin supplies on motorbikes to T1D children in Sri Lanka, India, Rwanda and Uganda.
 
So far this year we have dealt with many uncertainties and the need to make decisions about issues we could not have dreamed we would need to do. This is similar to the daily grind of glucose management and surprises faced by many families with childhood diabetes.
 
COVID fears have led to people avoiding hospitals and health providers. Will this have led to delayed diagnoses of new diabetes and more severe presentations? We know this has occurred in United States and Europe (Read "Unintended Consequences of COVID-19: Remember General Pediatrics"). How will the children and families manage returning to school? Will there be more obesity and has there been more fear preventing exercise? How has the social isolation affected the children and their mental health?

As a community ISPAD has a great role to play during the recovery. Even more than usual this is a time when we need to get-together to exchange ideas, science and friendship at our Annual Conference. As you know the 46th Annual Scientific Conference will be VIRTUAL (October 15-17, 2020). Save the date! More information will follow soon on 
2020.ispad.org. In the meantime, we encourage you to submit your abstracts and apply for one of the Support Grants here! The Support Grants are open to all ISPAD members under 40 and to all professional categories. Deadline is June 21. We hope that you will join this new and exciting adventure!
 
2020 is the year ISPAD marks 20 years of education with the Science Schools. I include a photo of the Third ISPAD Science School for Physicians in 2002 convened by Denis Daneman in Toronto.

Please be kind to yourselves and consider practising laughter (gelotology is the study of laughter and its effects on the body). May 3 was World Laughter Day!

Global Survey: Perceptions of Health care professionals worldwide

The aim of this survey is to identify different managements, knowledge and practice during COVID-19 pandemic in pediatric patients in all countries and to learn about many different views, so we can guide our future educational activities toward improving the management in this area. Indeed, we can learn a lot from each other by sharing our experience.

New Guideline “DKA in the time of COVID-19”

The new Clinical Practice Consensus Guideline highlights the use of subcutaneous insulin according to best evidence in the context of limited intensive care resources and potentially keeping children out of ICU to minimise risk of contracting the virus. This new Guideline was posted on ISPAD’s Forum and opened for comments until May 4.

ISPAD Clinical Practice Consensus Guidelines 2018

Chapter 4: The diagnosis and management of monogenic diabetes in children and adolescents
Monogenic diabetes is uncommon, accounting for approximately 1% to 6% of pediatric diabetes patients. One key indicator of monogenic diabetes is a family history of diabetes in one parent and first-degree relatives of that affected parent in patients who lack the characteristics of type 1 diabetes (no islet autoantibodies, low or no insulin requirements more than five years after diagnosis as well as a stimulated C-peptide >200 pmol/L). Another key indicator is the lack of characteristics of type 2 diabetes (marked obesity, acanthosis nigricans). Results of genetic testing should be reported and presented to families clearly and unambiguously, because results may have a significant effect on clinical management. Referral to a specialist in monogenic diabetes or an interested clinical genetics unit is recommended, where predictive testing of asymptomatic individuals is requested.

ISPAD-JDRF Research Fellowship 2020


ISPAD-JDRF Fellowship for ISPAD members below age 40, who wish to pursue a career in diabetes related research. It will cover USD 25,000 in research related expenses.
Expected project start: November 2020 - June 2021.

ISPAD Science Schools rescheduled to 2021

The COVID-19 Crisis has severely curtailed our ability to travel internationally. Consequently, the Science School for Physicians 2020 and Science School for Healthcare Professionals 2020 will be rescheduled to 2021. More details will be given in due course.

Diabetes & Ramadan

Ramadan is the Islamic holy month, a time of spiritual reflection, self-improvement, and heightened devotion and worship. Muslims fast from dawn to dusk during Ramadan and this can be challenging for a person with diabetes. ISPAD has devoted a chapter of their Clinical Practice Consensus Guidelines to this important topic: Fasting during Ramadan by young people with diabetes. Furthermore, EASD e-Learning Platform has developed different modules about managing diabetes during Ramadan.
