ISPAD NEWS                                                                          No.VI / June 2020

A message from ISPAD's President

For some of us, we are moving into Recovery Phase from the COVID-19 pandemic; many are still in the active phase. We need to adapt and adjust to the new normal, as we assist the children and their families navigate the consequences of social isolation. Thank you to all those who participated in the ISPAD survey on the coronavirus and diabetes.

ISPAD’s partnership with JDRF has enabled a nimble response for development of public awareness campaigns of coronavirus in the community, “Help keep our community safe: coronavirusdiabetes.org”. This guideline reached 75 million people through JDRF and Beyond Type1 and was taken up by 120 organisations. Over the next six months the executive including members from Brazil and India will continue membership of the Coronavirus-Diabetes Working Group.

Given the pandemic, the public Launch of the T1D Global index has been scheduled for World Diabetes Day in November. In the meantime, work continues with a range of experts to both quantify the impact of T1D in developed and developing countries, as well as formulate clear policy responses and calls to action to be announced alongside the index.

ISPAD Executive Board and Advisory Council are having virtual meetings during the American Diabetes Association virtual scientific sessions. We will be discussing ISPAD involvement in new education opportunities and planning future activities, including the 100 year anniversary of insulin development.

We hope you will join us in our ISPAD 2020 Virtual Conference on October 15-17. Please submit your abstracts by June 21 (more details on 2020.ispad.org/abstracts). Farid Mahmud is leading the ISPAD 2020 Virtual Program Organizing Committee, which builds on the Program for the planned 46th Annual Scientific Conference in Abu Dhabi developed by Asma Deeb and the Abu Dhabi Program Committee. Many thanks for the hard work from both committees. This is a new and exciting adventure.

2020 is the year ISPAD marks 20 years of education with the Science Schools. I include a photo of a recent ISPAD Science School for Physicians in 2018 convened by Tatsuhiko Urakami in Japan.

ISPAD Guidelines 2018 Highlighted Chapter

Cystic fibrosis-related diabetes (CFRD) is the most common comorbidity associated with cystic fibrosis (CF). CF is associated with a progressive deterioration in glucose tolerance. It can occur at any age, as early as infancy, and its prevalence increases as people with CF get older.
The earliest abnormality is often intermittent postprandial glucose excursions that can be detected by CGM, followed by indeterminate glycemia by OGTT, then impaired glucose tolerance (IGT) and finally, diabetes. With the progression of age, the prevalence of individuals with CF with normal glucose tolerance decreases. Intermittent postprandial hyperglycemia can often be detected by continuous glucose monitoring (CGM), even before fasting and two-hour oral glucose tolerance test (OGTT) glucose levels are abnormal.

The ISPAD Consensus Guideline on Management of CFRD in children and adolescents recommends that annual screening should begin at least by age ten in all people with CF who do not have CFRD. The use of HbA1c as a screening test for CFRD is not recommended.
People with CFRD should ideally be seen quarterly by a specialized multidisciplinary team with expertise in diabetes and CF and receive ongoing diabetes self-management education from diabetes education programs that meet national standards. CFRD is treated with insulin therapy. Oral diabetes agents are not as effective as insulin and are not recommended outside the context of clinical research trials.

If you want to know more about clinical practice issues on cystic fibrosis-related diabetes (CFRD) and dietary management, here is a presentation by Angela Matson.

We also recommend this presentation from ISPAD meeting 2015 by Charles Verge on how insulin deficiency appears in CFRF.
