A message from ISPAD's President

As many ISPAD members enjoy the Summer, in the Southern Hemisphere, it is Winter and thus our peak time for new diagnoses of type 1 diabetes.
A new diagnosis of diabetes brings much sadness to the family. Even more so when diabetes affects another sibling. This week I spoke to a family who was dealing with the diagnosis of new diabetes in a fourth child. Other siblings had been 2 years, 6 years, and 1.5 years at diagnosis of antibody positive diabetes, but this child was a baby not yet able to sit who was 9 months old. The mother told me her heart was broken. 
At the same time as trying to organise social support, this family reminds me of why we are trying so hard to improve diabetes care and prevention. I am reminded of how much we do not understand about the interplay of genetics and the environment, and how we need more research to find out why some children are at greater risk and how we need to slow down the process of beta cell loss in those at risk.

Conference Resource Platform - Highlighted Chapters

Epidemiology and Insulin Treatment

During the entire year 2019, we will facilitate your guidance through the ISPAD Guidelines 2018 by focusing at least once a month on a special chapter!

This month we are focusing on two chapters!

Chapter 1: Definition, epidemiology, diagnosis and classification of diabetes in children and adolescents
Increasingly, evidence suggests that trends by incidence of Type 1 diabetes vary markedly from country-to-country, by sex, race, age group as well as by time period around the world, consistent with disease etiology that involves environmental triggers super-imposed on generic susceptibility.

This chapter of the ISPAD Clinical Guidelines is a must-read for every clinician
it contains all necessary information about diagnosis, epidemiology and definitions, it informs what diagnostic tools may assist in confirming types of diabetes. The chapter specifies the differences between diagnosis and management of neonatal, monogenic, mitochondrial or cystic fibrosis related diabetes. This is extremely important to be aware of other, less common types of diabetes (neonatal, monogenic etc.), as it has important implications for both treatment and education.

Chapter 9: Insulin Treatment in Children and Adolescents with Diabetes
After almost 100 years since medicine was discovered, the dawn of diabetes technology has made new opportunities yet insulin still remains at the core of all recent advances in the treatment of children with diabetes.

This chapter highlights current pediatric recommendations for insulin injection and devices for insulin delivery. It provides updated advice for pediatric insulin dose adjustments and guidance on approaches for persistent deviations of glucose levels from target. By recommending what treatments to use, it highlights the importance of intensive regimes. It also discusses that every child diagnosed with diabetes is dependent on insulin for survival and therefore should have access to adequate amounts of at least regular and long acting (NPH) insulin.

, ISPAD is working toward making insulin available for all children and adolescents with diabetes and promoting universal insulin labeling.
