Required Documents for Membership Application

All Applicants: Upload a CV in English. Unless your profession, specialty, or affiliation clearly demonstrates your interest in diabetes, please make sure to provide additional documentation or information explaining your interest in pediatric diabetes.

Students, Fellows & Trainees: Upload proof of full-time enrollment. Team membership is not eligible.

Nurses, Educators, Dietitians & Healthcare Professionals: Upload a letter from Head of Department or a registration/practice certificate showing professional status.

Low/Lower-Middle/Upper-Middle Income Countries: Upload a recent document (e.g., utility bill) as Proof of Residency.

Patient Advocates: Complete an application form during signup. Patient Advocates who are physicians, nurses, or other healthcare professionals should apply under the appropriate professional membership category.

Team Membership: Hospitals and institutes can purchase a team membership for their diabetes team. A Physician, can apply for up to 5 team members from the same institution.
