• All Applicants: Upload a CV in English.

• Students, Fellows & Trainees: Upload proof of full-time enrollment. Team membership is not eligible. 

• Nurses, Educators, Dietitians & Healthcare Professionals: Upload a letter from Head of Department or a registration/practice certificate showing professional status. 

• Low/Lower-Middle/Upper-Middle Income Countries: Upload a recent document (e.g., utility bill) as Proof of Residency. 

• Patient Advocates: Complete an application form during signup.

• Team Membership: Hospitals and institutes can purchase a team membership for their diabetes team. A Physician, can apply for up to 5 team members from the same institution.
